PLEASE! Be sure to:
a) Complete the 'HEAD OF HOUSEHOLD FORM' once, and then
b) Complete a 'MEMBER OF HOUSEHOLD FORM' for EACH person:

a) Head of Household Form
(one per household)

b) Member of Household Form
(First, fill out and submit for the person designated as Head of Household, and
then again for EACH member of the household;
repeat as many times as needed for your household.)
